Certificates for common purposes

The clinic confirms the form, the checks involved and the fee when you book.

Work and work permits

For job applications, and for foreign nationals applying for or renewing a Thai work permit. The certificate must state that the applicant is not of unsound mind and does not have any of the prohibited diseases specified by the Department of Employment: leprosy, tuberculosis in a dangerous stage, elephantiasis at a stage with symptoms repugnant to society, serious drug addiction, chronic alcoholism and third-stage syphilis. Some offices ask for further checks. Workers of Cambodian, Lao, Myanmar and Vietnamese nationality must have their health check at a medical facility designated by the Department of Employment, so please check with the clinic before booking.

Study

For school and university applications. If the institution has its own form, please bring it to your appointment.

Thai driving licence

The Department of Land Transport form has two parts: you complete the health history, and the doctor records the examination. The certificate must be submitted within one month of the examination date.

Visas

For visa applications and extensions. Requirements vary with the visa type and the office handling it, so please mention the visa type when you book and bring any form you have been given. Some visa types and some embassies accept examinations only from a hospital or an approved doctor, so please check with the office handling your application before booking.

Sick leave

Issued after the doctor has examined you, stating what the examination found. See the general medical treatment page for more details.
